ACADEMIC NOTIFICATION NO. 01/2006

 

PROCEDURE FOR APPLYING FOR INTERNSHIP COMPLETION CERTIFICATE

FOR M.B.B.S. STUDENTS

 

1.         Respective college Dean/Principal of all affiliated Medical Colleges will forward the following documents of each student, who have completed his/her compulsory rotatory internship of requisite period as per Medical Council of India and / or University norms under a covering letter addressed to the Registrar Maharashtra University of Health Sciences, Nashik :-

a)    Original Internship Completion Certificate issued by College (Annexure I).

b)    Annexure III as per notification 3/2002 amended.

c)    Attested Xerox copy of Final Year Passing Certificate.

d)    Rs. 500/- crossed D.D. from Nationalised Bank, drawn in favour of the Registrar, Maharashtra University of Health Sciences payable at Nashik. (If not submitted along with convocation form)

2.         In case of loss of internship completion certificate issued by the University, a duplicate copy shall be issued by the University after submitting following documents by student / College.

(i)   A crossed D.D. of Rs. 500/- from Nationalised Bank, drawn in favour of the Registrar, Maharashtra University of Health Sciences, payable at Nashik.

(ii)   An original copy of affidavit stating the loss of internship completion certificate and will submit the same if found in future to the University

(iii) Attested xerox copies of mark sheet and passing certificate.    

3.         Incomplete proposals will not be processed.

FOR B.D.S. STUDENTS

 

1.         Respective college Dean/Principal of all affiliated Dental Colleges will forward the following documents of each student, who have completed his/her compulsory rotatory internship of requisite period as per Dental Council of India and / or University norms under a covering letter addressed to the Registrar Maharashtra University of Health Sciences, Nashik :-

(A)      Original Internship completion certificate issued by college (Annexure-I)

(B)       Original Rotatory Internship training programme. (Annexure-II)

(C)       Attested Xerox copy of final year marksheet 

(D)      Attested xerox copy of final year passing certificate.

(E)       Rs. 500/- crossed D.D. from Nationalised Bank, drawn in favour of the Registrar, Maharashtra University of Health Sciences payable at Nashik. (If not submitted along with convocation form)

2.         In case of loss of internship completion certificate issued by the University, a duplicate copy shall be issued by the University after submitting following documents by student / College.

(i)   A crossed D.D. of Rs. 500/- from Nationalised Bank, drawn in favour of the Registrar, Maharashtra University of Health Sciences, payable at Nashik.

(ii)   An original copy of affidavit stating the loss of internship completion certificate and will submit the same if found in future to the University

(iii) Attested xerox copies of mark sheet and passing certificate.

1.         Respective college Dean/Principal of all affiliated Ayurved & Unani Colleges will forward the following documents of each student, who have completed his/her compulsory rotatory internship of requisite period as per Central Council of Indian Medicine and / or University norms under a covering letter addressed to the Registrar Maharashtra University of Health Sciences, Nashik :-

(a)                Original Internship completion certificate issued by college. (Appendix – B)

(b)                Attested Xerox copy of final year marksheet.

(c)                Attested xerox copy of final year passing certificate.

(d)               Rs. 500/- crossed D.D. from Nationalised Bank, drawn in favour of the Registrar, Maharashtra University of Health Sciences payable at Nashik. (If not submitted along with convocation form)

(a)                Original Internship completion certificate issued by college.

(b)               Attested Xerox copy of final year marksheet.

(c)                Attested xerox copy of final year passing certificate.
